We’ll discuss any questions or issues you may have and provide tailored treatment options. Includes evaluating your teeth, gums, bite, and oral health overall:
- Teeth condition, including damage, decay, or missing teeth.
- Early indicators of cavities.
- Gum health, checking for inflammation or other signs of gum disease that could lead to tooth or bone loss.
- Assessment of previous dental work such as fillings, crowns, or root canals to ensure they remain in good condition.
- Screening for potential signs of oral cancer, including unusual growths, cysts, or abnormalities.
- Examination of the tongue, roof, and floor of the mouth for signs of bleeding or inflammation.
- Evaluation of tooth alignment, spacing, and bite.
- Identification of clenching or teeth grinding, which can cause jaw soreness, headaches, tooth or hearing loss if untreated.
- Assessment of the temporomandibular joint (TMJ) for pain, tenderness, or dysfunction.

If available, you can request X-rays from your previous dentist. These can be compared with new ones to provide a clearer understanding of the evolution of your oral health.

The Patient Intake Form and Medical History provide us with all the relevant personal and medical information needed for our providers to gain a comprehensive understanding of your needs and condition, such as a list of medications you’re currently taking. A link to the digital form will be sent by email after your appointment is scheduled. Alternatively, if you prefer, a printed version can be completed in person.
